The high fat content in Western diets probably affects placental function during pregnancy with potential consequences for the offspring in the short and long term. The aim of the present study was to compare genome-wide placental gene expression between rat dams fed a high-fat diet (HFD) and those fed a control diet for 3 weeks before conception and during gestation. Gene expression was measured by microarray and pathway analysis was performed. Gene expression differences were replicated by real-time PCR and protein expression was assessed by Western blot analysis. Placental and fetal weights at E17.25 were not altered by exposure to the maternal HFD. Gene pathways targeting placental growth, blood supply and chemokine signalling were up-regulated in the placentae of dams fed the HFD. The up-regulation in messenger RNA expression for five genes Ptgs2 (fatty acid cyclo-oxidase 2; COX2), Limk1 (LIM domain kinase 1), Pla2g2a (phospholipase A2), Itga1 (integrin α-1) and Serpine1 was confirmed by real-time PCR. Placental protein expression for COX2 and LIMK was also increased in HFD-fed dams. In conclusion, maternal HFD feeding alters placental gene expression patterns of placental growth and blood supply and specifically increases the expression of genes involved in arachidonic acid and PG metabolism. These changes indicate a placental response to the altered maternal metabolic environment.
Developing National Standards for Public Health Emergency Preparedness With a Limited Evidence Base

Objective: The paucity of evidence and wide variation among communities creates challenges for developing congressionally mandated national performance standards for public health preparedness. Using countermeasure dispensing as an example, we present an approach for developing standards that balances national uniformity and local flexibility, consistent with the quality of evidence available.
Methods: We used multiple methods, including a survey of community practices, mathematical modeling, and expert panel discussion.
Results: The article presents recommended dispensing standards, along with a general framework that can be used to analyze tradeoffs involved in developing other preparedness standards.
Conclusions: Standards can be developed using existing evidence, but would be helped immensely by a stronger evidence base.

Introduction
Public–private partnership overview
Many developing and developed countries around the world face the challenge of meeting their growing infrastructure needs with limited fiscal resources. It is estimated that worldwide population will increase by approximately one billion between 2005 and 2015 (Sachs 2005). Large-scale development and maintenance of infrastructure will be needed to accommodate the basic needs of these new inhabitants, as well as those of the existing population. However, many developing countries receive less financial support from the World Bank in recent years than they did during the 1960s and 1970s for infrastructure development as a result of the multilateral agencies' shift in focus from infrastructure development to social program development during the 1980s (Harris 2003). At the same time, developed nations such as the United States have found it difficult to raise taxes to maintain their existing infrastructure systems, especially amid the economic crisis of the recent 2000s. Years of delayed or cancelled infrastructure maintenance projects led the American Society of Civil Engineers (ASCE) to give a grade “D” to America's existing infrastructure in 2009 (ASCE 2009). It is estimated that $2.2 trillion of investment will be needed for the next 5 years to fix America's infrastructure (ASCE 2009).
In response to these challenges, some governments have invited private investors to participate in infrastructure development through schemes such as Public–Private Partnership (PPPs), Private Finance Initiative (PFI), and Build–Operate–Transfer (BOT) (Clarke 2000; Gerrard 2001; Lonsdale 2005).

Summary
Ablative laser resurfacing has been considered the most effective treatment option for skin rejuvenation. However, the epidermis is significantly damaged during this process, and this can be associated with potential adverse effects, including transient erythema, pigmentary disturbances, infection, and scarring, especially in Asians (Nanni and Alster 1998). Nonablative skin resurfacing involves selective thermal damage to a subepidermal layer. It is achieved by a combination of laser radiation reaching the dermis and the concurrent use of cooling to protect the epidermis. Although nonablative techniques are associated with less downtime, the degree of improvement appears to be limited (Chan and Kono 2004).
On the other hand, recent advances in fillers and neurotoxins have allowed the use of these agents in Asians, with minimal complications. Both fillers and neurotoxins are effective in producing an optimal cosmetic result. Currently the use of fillers and neurotoxins ranks as the most commonly performed cosmetic procedure in Asia.
Patient selection: differences and similarities from Caucasians to Asians
The demand for wrinkle reduction is very high, and patient selection is similar to patient selection for Caucasians. The efficacy and complication rate of fillers and neurotoxins in Asians are also similar to those in Caucasians (Nanni and Alster 1998). However, there are some histologic differences between Asian skin and Caucasian skin, for example, Asian skin tends to have a thicker dermis with more abundant collagen fiber.
With regard to the injection of fillers, certain areas, such as the nasolabial fold and tear trough, can be tricky.

The objective of the present paper is to provide glycaemic index (GI) and glycaemic load (GL) values for a variety of foods that are commonly consumed in Hong Kong and expand on the international GI table of Chinese foods. Fasted healthy subjects were given 50 g of available carbohydrate servings of a glucose reference, which was tested twice, and test foods of various brands of noodles (n 5), instant cereals (n 3) and breads (n 2), which were tested once, on separate occasions. For each test food, tests were repeated in ten healthy subjects. Capillary blood glucose was measured via finger-prick samples in fasting subjects ( − 5, 0 min) and at 15, 30, 45, 60, 90 and 120 min after the consumption of each test food. The GI of each test food was calculated geometrically by expressing the incremental area under the blood glucose response curve (IAUC) of each test food as a percentage of each subject's average IAUC for the reference food. GL was calculated as the product of the test food's GI and the amount of available carbohydrate in a reference serving size. The majority of GI values of foods tested were medium (a GI value of 56–69) to high (a GI value of 70 or more) and compared well with previously published values. More importantly, our dataset provides GI values of ten foods previously untested and presents values for foods commonly consumed in Hong Kong.

The Influence of the Composition of the Nursing Staff on Primary Bloodstream Infection Rates in a Surgical Intensive Care Unit

Objectives:
To determine the risk factors for acquisition of nosocomial primary bloodstream infections (BSIs), including the effect of nursing-staff levels, in surgical intensive care unit (SICU) patients.
Design:A nested case-control study.
Setting:A 20-bed SICU in a 1,000-bed inner-city public hospital.
Patients:28 patients with BSI (case-patients) were compared to 99 randomly selected patients (controls) hospitalized ≥3 days in the same unit.
Results:Case- and control-patients were similar in age, severity of illness, and type of central venous catheter (CVC) used. Case-patients were significantly more likely than controls to be hospitalized during a 5-month period that had lower regular-nurse-to-patient and higher pool-nurse-to-patient ratios than during an 8-month reference period; to be in the SICU for a longer period of time; to be mechanically ventilated longer; to receive more antimicrobials and total parenteral nutrition; to have more CVC days; or to die. Case-patients had significantly lower regular-nurse-to-patient and higher pool-nurse-to-patient ratios for the 3 days before BSI than controls. In multivariate analyses, admission during a period of higher pool-nurse-to-patient ratio (odds ratio [OR]=3.8), total parenteral nutrition (OR=1.3), and CVC days (OR=1.1) remained independent BSI risk factors.
Conclusions:Our data suggest that, in addition to other factors, nurse staffing composition (ie, pool-nurse-to-patient ratio) may be related to primary BSI risk. Patterns in intensive care unit nurse staffing should be monitored to assess their impact on nosocomial infection rates. This may be particularly important in an era of cost containment and healthcare reform.

During 1999 alone several conferences and seminars were dedicated to identities in Australia: “Alter/Asians: Exploring Asian/Australian Identities, Cultures and Politics in an Age of Crisis” held in Sydney in February, the one-day conference “Cultural Passports” on the concept and representations of “home” held at the University of Sydney in June, and “Asian-Australian Identities: The Asian Diaspora in Australia” at the Australian National University in September. To me as a Chinese who had his childhood and education in New Zealand this concern with identity is not exceptional: I remain a keen reader of New Zealand fiction and poetry in which Pakeha New Zealanders have agonised and problematised their search for identity as an island people living among an aggressive indigenous population and in an insecure dependent economy. New Zealand identity has always been problematised as has Chinese identity: what does it mean to be Chinese? Now Asian identity has become the current issue: “We're not Asians” was the title of the paper by Lily Kong on identity among Singaporean students in Australia. White Australians appear much more content and complacent with their identity and do not indulge as much in navel gazing. And yet it may be that it is the “Australian identity” that needs to be challenged and contested so that it becomes less an exclusively WASP-ish male mateship and more inclusive of women, Aborigines and Asians.
Detecting postnatal depression in Chinese women: Validation of the Chinese version of the Edinburgh Postnatal Depression Scale

Background
We evaluated the utility of the Chinese version of the Edinburgh Postnatal Depression Scale (EPDS) and measured the prevalence of major depression six weeks after confinement among Chinese women in Hong Kong.
MethodA prospective cohort of 145 women completed the EPDS, the 12-item General Health Questionnaire (GHQ) and the Beck Depression Inventory (BDI) six weeks after giving birth. They were then assessed with the Structured Clinical Interview for DSM–III–R, non-patient version (SCID–NP) to establish psychiatric diagnosis. The criterion validity of EPDS was tested against this clinical diagnosis, and the concurrent validity against the GHQ and BDI scores was also evaluated. The internal consistency of the scales was measured by Cronbach's α coefficient.
ResultsThe Chinese EPDS had satisfactory psychometric properties and a cut-off score of 9/10 is recommended for screening depressive illness in a general postnatal population. At six weeks postpartum, 5.5% of the study population suffered from major depression.
ConclusionsThe Chinese EPDS will be useful for screening for postnatal depression.
